Online Patient Registration

In order to streamline your experience in our office, you may preregister with our office by filling out our Secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. This will save you valuable time at your first visit. You may also print the forms and bring to the office. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Financial

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.

We also work with Care Credit, a finance option that allows payment to be spread out over many months. If this option is appealing to you, please click on the Financial link below to obtain additional information or you can contact our staff.
